About the Role
*Overview*
We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality healthcare services to patients in their homes. The ideal candidate will possess a strong clinical background and demonstrate excellent patient assessment skills. This role is essential in ensuring that patients receive the necessary care and support in a comfortable environment, promoting their overall well-being and recovery.
We are looking to hire a Registered Nurse to join our team, to conduct IN-HOME assessments in Orange and Dutchess Counties.
This is a per-diem position, based on amount of assessments we have available that month. No minimum amount of hours/visits guaranteed.
Job description:
The RN would be working under the direction of the DPS (director of patient services) to evaluate client’s in their homes.
These visits are to assess the client’s homecare needs, create or adjust a Plan of care, and oversee that the caregiver is implementing the plan of care appropriately.
Instructs and advise patients and/or families as well as the home health aides in their care.
Reassessments would be assigned in the beginning of each month and should be scheduled by the nurse according to both her and the client’s availability.
A list of aide annual supervisions would be assigned at the beginning of the month.
Start of care assessments are on a non-predictable schedule, but need to be done within 24 hours of the start of care.
PRN visits if there is a change of condition that requires a nurse’s follow up.
The nurse will conduct in office Orientations to all new caregivers in a group setting, on a predetermined schedule, in our Newburg Office.
The nurse is responsible to submit to the DPS all their documentation from their visits within 24 hours of the visit.
The nurse needs to communicate to the DPS any concerns or client changes.
In addition, there is an occasional need to conduct visits in Rockland County when the Rockland nurses are out.
Requirements
Please have the following:
Valid and current New York State RN license and registration.
Active driver’s license and reliable transportation.
Clear English
Strong assessment skills.
Able to work independently and as part of a team.
Have good oral and written communications skills.
Previous homecare or assessment nursing experience preferred.
Updated physical exam
Up to date with all immunizations
PPD
