Eligibility Criteria for the NHTD Waiver Program
 

  • To qualify for the NHTD Waiver Program, individuals must meet the following criteria:

  • Be Medicaid-eligible in New York State

  • Have a Patient Review Instrument (PRI) completed to assess their long-term care needs

  • Require either Custodial or Nursing Home Level of Care

  • Be able to live safely within a community setting with the support of waiver services
